New Patients Information

The staff and doctors at SFPMG are looking forward to meeting you, and we will do everything for you to have a pleasant and relaxed experience while you visit us.

On the day of your first visit we kindly request to come in 15 minutes prior to your appointment, and bring along any recent laboratory test results and other pertinent medical information with you. Also, bring along the supplements and prescription drugs you are currently taking.

Patient visit costs vary depending on the doctor you are choosing. Lab work and other tests may incur additional fees. Some specialty laboratories do not offer direct insurance billing. All fees are due and payable at the time of your visit. We do not bill insurance directly, but we do provide a complete itemized statement to file with your insurance for your reimbursement.

Please note that we have a 48 hour cancellation policy. We spend much time with a patient, so we kindly request to give us plenty of notice of cancellations. We will attempt to fill cancelled appointments, and no charges will be made if successful. You are responsible for associated charges if the cancellation occurs less than 48 hours in advance and we are unable to fill the cancellation. For this reason, we request a valid credit card number at the time of scheduling in order to secure your appointment.
